PD61 What Are The Drivers Of Transitioning From Fee-For-Service To Value-Based Payment Models In The Brazilian Private Healthcare System?

Introduction The Brazilian National Agency for private healthcare system (ANS) makes the regulation in Brazil. ANS, since 2019, is running pilot value-based new payment models project. In total, 13 projects were selected by ANS. This research aims to identify key drivers moving from fee service (FFS) system. Methods We interviewed managers of plans (13 total) participating Value-Based Payment Models run Data collected through semi-structured interviews during 2021. Twelve invited interview and eight accepted invitation. questions were: “Why are providers transitioning model models?” “What your motivations participate ANS project?” For data analysis, Bardin’s content analysis was chosen. validation performed using debriefing technique. Results main reasons FFS related weaknesses (58%), strengths (14%) sector needs (14%). Fee-for-service financial impacts – including waste unsustainability (55%), lack transparency trust conflict interest (28%). Strengths benefits (100%), other words, greater return on investment. unmet improvement status - lower costs less (71%), care delivery quality (29%). Continuity reported as a benefit FFS, according 43 percent respondents. Conclusions Our results suggest that reason transition fee-for-service models.
